You will be able to perform the following activities when logged in:
1. Download a copy of Matriculation Oath Form. Fill the form and scanned to PDF format
2. Download a copy of Affidavit Form. Fill the form appropriately and scanned to PDF format
3. Upload the scanned copy of each of the forms sepatately by clicking on the appropriate upload button
4. You will need to produce the original copy of the uploaded forms when the school resumes for physical activities
